Just got back from Peru. This is the first time I have ever visited South America. It is hard to go there because we don't speak Spanish and the infrastructure is not as good as Europe, but it is so worth it.

Our itinerary:
Day 1: Fly to Lima.
Day 2: Lima sightseeing.
Day 3: Lima sightseeing.
Day 4: Fly to Cusco (11,000 ft.) in the morning, rest to get used to the high altitude.
Day 5: Cusco sightseeing.
Day 6: Visit Saqsaywaman, Pisac etc. by taxi, sleep in Urubamba.
Day 7: Visit Chinchero, Maras, Moray by taxi, sleep in Ollantaytambo.
Day 8: Ollantaytambo sightseeing, go to Aguas Calientes by train in the afternoon.
Day 9: Visit Machu Picchu, leave for Cusco by train in the afternoon.
Day 10: Relax in Cusco; fly to Lima then U.S. at night.
Day 11: Arrive home.

I am quite happy with the itinerary, although you could shorten the Lima stay by one day if you are short on time.
