Minor failures tend to slide into negligent
small lessons are not learned to prevent big failures
and catastrophic ones are just standing-by
Out minds get occupied by too many things
multifaceted virtue is too much to ask
and the flux of organization encourage no consistency
because of emphasis on changes, adaptions
Leadership changes just as often as one switches partner
so the flux is expected to listen to the flow?
Perhaps failures are well regarded as one of daily rituals
or exercises to be dealt with
they have really become no big deal
Instead of going after perfection, against human nature
just take it as necessity as part of life
to be managed in a mind set of probability
as long as it is not a total blown-off
